ALL ATHLETES MUST COMPLETE THE PARENT SIGN OUT FORM 48 HOURS PRIOR TO THE GAME IN ORDER TO LEAVE WITH YOUR PARENT OR GUARDIAN ONCE THE GAME IS OVER. THE FORM MUST BE TURNED IN HARD COPY TO THE ATHLETIC DIRECTOR.
